Agent 17 med kodenavn "Otto" trænger ind fra taget på en 30 etager høj bygning. Formålet er at bevæge sig ned til kælderen, primært ved brug af elevatorer. Undervejs skal der opsamles alle hemmelige dokumenter, som gemmer sig bag røde døre. Fra blå døre kommer fjendtlige agenter, der skal undgåes eller uskadeliggøres, før man selv bliver uskadeliggjort. Ti år efter udgivelsen lavede Taito en opfølger i form af Elevator Action Returns.
